Case Brief Summary & Legal Analysis
tl;dr: The Supreme Court upheld the Affordable Care Act’s individual mandate as a constitutional tax, but not as a regulation of commerce. It also found the law’s mandatory Medicaid expansion unconstitutionally coercive, making it a voluntary option for states by limiting the federal government’s enforcement power.
Legal Significance: This landmark decision established a key limit on the Commerce Clause, holding it cannot compel commercial activity. It also revitalized the anti-coercion doctrine under the Spending Clause, while simultaneously affirming a broad congressional taxing power for regulatory purposes through constitutional avoidance.

Case Facts & Court Holding
Key Facts & Case Background
The Patient Protection and Affordable Care Act of 2010 (ACA) contained two central provisions challenged in this case. First, the “individual mandate” required most Americans to maintain “minimum essential” health insurance coverage or make a “shared responsibility payment” to the Internal Revenue Service (IRS). Congress asserted this was necessary to support insurance market reforms, such as prohibiting insurers from denying coverage for pre-existing conditions, by preventing healthy individuals from waiting until they were sick to purchase insurance. Second, the “Medicaid expansion” required states to expand their programs to cover all adults with incomes up to 133% of the federal poverty level. The ACA stipulated that if a state refused to comply with the expansion, the federal government could withhold not only the new funds for the expansion but all of its pre-existing federal Medicaid funding. For most states, this funding represented over 10% of their total budget. A coalition of 26 states, the National Federation of Independent Business (NFIB), and several individuals sued, arguing that both provisions exceeded Congress’s enumerated powers under the Constitution.
Court Holding & Legal Precedent
Issue: Does Congress have the constitutional authority under the Commerce Clause or the Taxing Power to require individuals to purchase health insurance, and does the Spending Clause permit Congress to condition a state’s entire existing Medicaid funding on its acceptance of a significant program expansion?
